Trinity Hospice provides the coordinated services of a multi-disciplinary team of professionals and trained volunteers through a personalized plan of care. Under the direction of a patient's physician and the hospice medical director, the hospice team works together to provide physical, emotional and spiritual care to the patient and their family. The Trinity Hospice team works with your religious representative, as requested, to ensure that the religious resources of your traditions are available during the terminal illness. Additionally, Trinity Hospice is pleased to offer a special program of care for residents of long-term care facilities who are facing a terminal illness. In partnership with the long-term care facility staff, the hospice team provides state-of-the-art symptom management, as well as emotional and spiritual support for residents and families.
